dc.description.abstractHepatoselüler karsinomda (HCC) tümörün yinelemesi önemli bir prognostik belirteçtir. Özellikle de HCC nedeniyle karaciğer transplantasyonu yapılan hastalarda en önemli ölüm nedenlerinden biridir. Yineleyecek olguların önceden bilinmesi, yakın takip ve tedavi açısından oldukça önemlidir. Morfometrik yöntemler ile değerlendirilen çekirdek özellikleri çoğu kanserde kullanışlı prognostik öngörücüler olarak bildirilmiştir. Bu çalışmada amaç, HCC’nin biyolojik davranışını belirleyen bazı histopatolojik ve morfometrik özellikleri, HCC nedeniyle karaciğer transplantasyonu sonrasında HCC’nin yinelediği (Grup1) ve yinelemediği (Grup2) gruplar arasında karşılaştırmaktır. HCC nedeniyle karaciğer transplantasyonu yapılan ve 6 tanesinde tümör yinelemesi görülen 37 hastanın hepatektomi materyallerine ait arşiv preparatları ve dosya bilgileri tekrar incelendi. Her olguda; yaş, cinsiyet, tümör odağı sayısı, en büyük odak çapı, etiyoloji, tümör sınırları, tümör ve komşuluğunda inflamasyon, patern, nekroz, özel hücre tipi, yağlı değişiklik, Mallory cisimciği, çekirdekte inklüzyon, mikrovasküler invazyon ve mitoz yeniden değerlendirildi. Her olgudan en az 50 tümör hücresi içerecek şekilde, x1000 büyük büyütmede, en az 5 adet alanın fotoğrafı çekildi. Her alanda seçilebilen tüm hücrelerin çekirdekleri ImageJ (1.40) programındaki kalem aracı kullanılarak belirlendi ve çekirdek çevresi, alanı ve yuvarlaklık yüzdesi hesaplandı. Parametrik testlerin ön şartlarını yerine getiren değişkenler için iki grup ortalamasının karşılaştırılmasında Student’s t testi, ikiden fazla grup ortalamalarının karşılaştırmasında ise Tek Yönlü Varyans Analizi yöntemi (One Way ANOVA) kullanıldı. Sonuçlar ortalama±standart sapma olarak ifade edildi. Kategorik değişkenlerin analizinde Pearson ki-kare testi, Olabilirlik oran testi ve Fisher-Exact test kullanıldı. Sonuçlar n (%) olarak ifade edildi. p<0.05 düzeyi istatistiksel olarak anlamlı kabul edildi. Veri setinin analizinde SPSS 13.0 (Statistical Package for the Social Sciences, version 13.0, SSPS Inc, Chicago, IL, USA) istatistik paket programı kullanıldı. Sonuçlara bakıldığında Grup 1’de az diferansiye tümör oranının anlamlı olarak Grup 2’den fazla olduğu (p=0.042) görüldü. Bunun yanı sıra anlamlı olmamakla birlikte Grup 1’de mikrovasküler invazyon ve mitoz sayısı ortalaması Grup 2’ye göre daha fazladır. Grup 2’de ise kolestaz, Mallory cisimciği, nekroz, tümörde inflamatuvar infiltrat ve yağlı değişiklik daha fazladır. Morfometrik özellikler her iki grupta benzer oranda saptanmıştır. İlintilendirmelerin çoğu istatistiksel olarak anlamlı değildir. Bu da çalışmadaki yineleyen HCC sayısının azlığına bağlı olabilir. Literatürde artmış mitotik aktivite, tümörde yüksek grade ve mikrovasküler invazyonun yineleyen HCC için öngörücü faktörler olabileceği bildirilmektedir. Az sayıda çalışmada ise patolojik inceleme ile yinelemenin öngörülemeyeceği belirtilmiştir. HCC nin grade’i nükleer morfometrik özellikleri de içermektedir. Bu çalışmada; tümör yinelemesinin hücre morfolojisi dışındaki faktörlerle ilişkili olabileceği hipotezi desteklenmektedir. Recurrence of hepatocellular carcinoma (HCC) is an important prognostic marker. It is one of the most important risk factors in mortality of the patients who had liver transplantation for this reason. The prediction of a possible recurrence of the tumor is pretty important for the close follow-up and medical therapy of these patients. The nuclear parameters that can be measured by morphometric indices were reported in the literature to be the predictors of prognosis in other types of tumors. The data achieved from computered image analysis systems are known to be either objective or easy to obtain by a conventional microscopic analysis. The aim of this study is to compare some of the histopathologic and morphologic features that may determine the biologic behaviour of HCC, between recurrent (Group1) and nonrecurrent (Group2) groups. The hospital files and hepatectomy slides of 37 patients who had a liver transplantation because of HCC were examined again. Six of these patients were known to develop recurrence of HCC. Age, sex, number number, largest tumor size, etiology, tumor edges, tumor and background inflammation, architecture, necrosis, cell types, fatty change, Mallory bodies, nuclear inclusion, microvascular invasion and mitoses are reevaluated in each case. Five images that contain at least 50 tumor cells -enlarged x1000 times under light microscope- were obtained for each case. All the nuclear contours on each image were defined by the pencil tool of ImageJ (1.40) programme. The nuclear area, perimeter and circularity of each cell were calculated by the same programme. Student’s t test, One Way ANOVA, Pearson Chi-square test and Fisher-Exact tests were used in statistical analysis. The results were given in n (%). p<0.05 was accepted as scientifically significant. SPSS 13.0 (Statistical Package for the Social Sciences, version 13.0, SSPS Inc, Chicago, IL, USA) statistical programme was used in the analysis of our data. Poor differentiation was scientifically significant (p=0.042) in Group 1. Although they were not scientifically significant, microvascular invasion and increased mitotiv activity were detected to be more prominent in Group 1. Again although not scientifically significant, cholestasis, Mallory bodies, necrosis, tumor inflammation and fatty change were the features in Group 2 that were detected to be more prominent when compared to Group 1. Both of the groups were detected to have similar morphometric results. These results can be explained with the limited number of cases -especially recurrent HCC group-in the study. Increased mitotic activity, higher tumor grade and higher microvascular invasion rates were reported in the literature to be strong possible predictors of HCC that recur. There are also rare studies in the literature reported that pathologic studies are unable to predict the recurrence of HCC. The grade of HCC also contains the nuclear morphometric features. In this study, the hypothesis of “factors other than cell morphology are prone to have relation with tumor recurrence” was supported by the way.en_US
