Alt ektremitede kronik safen ve perforan ven yetmezliği bulunan hastalarda endovenöz lazer tedavisinin etkinliğinin değerlendirilmesi
Özet
Kronik venöz yetmezlik, toplumda oldukça yaygın olarak izlenen bir hastalık olup, venöz
kapakçıkların fonksiyonunu yitirmesine bağlı kalbe venöz dönüsün azalması ile venöz
sistemde basınç yükselmesi ile karakterizedir. Çoğunlukla variköz genislemeler olaya eslik
eder. Bu hastalarda geleneksel tedavi yöntemi olan cerrahi yaklasımın alternatifi olarak,
endovenöz lazer tedavisi (EVLT) son yıllarda giderek artan bir sıklıkta uygulanmaktadır.
Nispeten yani sayılabilecek bu tedavi yönteminde, renkli Doppler ultrasonografide
(RDUS) yetmezlik bulguları saptanan safen ya da perforan venlere ultrason esliğinde
yerlestirilen intravasküler lazer probu ile bu damarlarda geri dönüsümsüz hasar
olusturulmakta ve bu sayede yetmezliğe sekonder gelisen semptomlar ortadan
kaldırılabilmektedir.
Alt ekstremitelerde varis ve/veya ağrı, sislik sikâyetleriyle Girisimsel Radyoloji ünitesine
basvuran ve RDUS ile değerlendirilmesini takiben kronik safenöz ve/veya perforator venöz
yetmezlik bulguları saptanan hastalardan, akut derin ven trombozu, belirgin sistemik
hastalığı, ciddi alt ekstremite arteriel hastalığı, karaciğer yetmezliği, lokal anesteziklere
bilinen belirgin ilaç alerjisi, koagülasyon bozukluğu olmayan, islem yapılacak bölgede
aktif cilt enfeksiyonu bulunmayan ve gebe olmayan 50 hastada, 71 bacakta, 90 damara
EVLT uygulandı. Hastalara islem ile ilgili gerekli bilgilerin verilmesi ve onam formlarının
imzalatılmasını takiben her bir hastanın tedavi uygulanacak ekstremite veya
ekstremitelerinin venöz RDUS bulguları bir formda toplandı. EVLT islemleri 980 nm
dalga boylu, 400 mikron fiberoptik baslıklı cihaz ile gerçeklestirildi. Ayrıca gerekli olan
olgularda tedaviye skleroterapi de eklendi. Đslem sonrasında hastalara 1. hafta, 1. ay, 3. ay,
6. ayda rutin, sikâyetleri olduğunda ise aynı gün içinde fizik muayene ve RDUS kontrolü
yapıldı. Đsleme bağlı olarak 3 hastada major, 45 hastada minör komplikasyon gelisti.
(rakamlar en sık görülen komplikasyonlara göre verilmistir) Bes-yirmi aylık takipler
sonrasında lazer uygulanan 90 damarın % 87’sinde kalıcı tam oklüzyon, %6.5
semptomatik, %6.5 asemptomatik rekanalizasyon saptandı. Sonuç olarak, kronik safen ya
da perforan ven yetmezliği olan hastaların tedavisinde EVLT, cerrahiye alternatif olarak
uygulanabilecek, etkili ve güvenli bir tedavi yöntemidir.
Chronic venous insufficiency is a common disease in the general population where venous
valves lose their function resulting into reduction of venous return to the heart along with
the venous pressure rise in the systemic circulation accompanying frequent varicose
enlargements. In this group of patients, surgical treatment is traditional method where
endovenous laser treatment (EVLT) is an alternative treatment which is being used
increasingly in recent years. With this comperatively new method, saphenous or
perforating veins with findings of venous failure identified by color Doppler
ultrasonography (CDUS) are punctured along with ultrasound and irreversible damage is
obtained by laser in those veins thus treating the underlying symptoms secondarily
associated with failure.
EVLT is applied to 90 vessels in 71 legs of 50 patients who has no findings of acute deep
vein thrombosis, significant systemic disease, severe lower extremity arteryal disease, liver
failure, known significant drug alergy to anesthetics or coagulation disorder and without
any active skin infection in procedure region and without pregnancy who were referred to
interventional radiology department with varice and/or pain and swelling complaints in
lover extremities evaluated with color Doppler ultrasonography with findings of chronic
saphenous or perforating venous insufficiency. After giving necessary informations about
the procedure for each of patients and having signed consent forms, venous color Doppler
ultrasonography findings of each treatment planned extremities were collected in a form.
EVLT device used in this procedure is a 910 nm wavelength 400 microns fiberoptic
capped device titled as FOX brand. Sclerotherapy with 2 %polidacanol (aethoxysklerol) is
also added to the treatment in cases where it is needed. All the patients are controlled with
color Doppler ultrasonography routinely in 1th week, 1th month, 3th months and 6th
months of the post-procedural period where same day color Doppler ultrasonography is
applied if any complaints are present. 3 major and 45 minor complications are occured in
patients due to the procedure. (Figures are given related to the most common
complications). After 5-20 months of follow-up, permanent total occlusion in 87% , partial
permanent occlusion in 89% and asymptomatic recanalisation in 44% of the vessels are
observed out of 90 vessels where laser is applied.
As a result, EVLT is an effective and safe treatment method alternative to surgery in
patients with chronic saphenous and perforating vein insufficiency
