dc.description.abstractHashimoto tiroiditi (HT), sık görülen otoimmün tiroid hastalığı olup, iç kulak dahil herhangi bir organda sistemik etkisi olabileceği iddia edilmektedir. Yapılan çalışmalarda Meniere hastalığı (MH) ve benign paroksismal pozisyonel vertigo (BPPV) tanısı olan hastalarda otoimmün tiroid hastalığı sıklığında artış saptanmış ve her iki hastalığın etiyopatogenezinde otoimmünite suçlanmıştır. Bu çalışmanın amacı; Hashimoto tiroiditi olan hastalarda vestibüler fonksiyonları ve levotiroksin (LT4) tedavisinin etkisini değerlendirmektir. Araştırma Başkent Üniversitesi Tıp Fakültesi Endokrinoloji ve Metabolizma Hastalıkları ve Kulak Burun Boğaz Hastalıkları polikniklerinde Mart 2018 - Ağustos 2018 tarihleri arasında yürütülmüştür. Çalışmaya 18-60 yaş arası, her iki cinsiyetden, baş dönmesi, kulak çınlaması ve işitme kaybı gibi vestibüler sisteme ait yakınması olmayan ve HT tanısı olan toplam 61 hasta ve 30 sağlıklı birey alınmıştır. Hasta bireyler LT4 tedavisi alan (30 hasta) ve almayan (31 hasta) olmakla 2 grupta değerlendirilmiştir. Tüm bireylerin serbest T3, serbest T4, TSH, Anti-TPO ve Anti-Tg düzeyleri ölçülmüştür ve Başkent Üniversitesi Kulak Burun Boğaz Kliniği Vestibüler Laboratuarında servikal vestibüler uyarılmış miyojenik potansiyeller (cVEMP), oküler vestibüler uyarılmış miyojenik potansiyeller (oVEMP) ve video baş itme testi (vHİT) testleri yapılmıştır. Çalışmada cVEMP testlerindeki anormalliğin, HT olan hastalarda daha sık olduğu görüldü, ancak gruplar arası anlamlı farklılık olmadığını tespit ettik. oVEMP testleri ise her iki grupta eşit sayıda anormaldir ve oVEMP sonuçlarında da anlamlı farklılık elde etmedik. Çalışmamızda vHİT testleri ile her üç SSK değerlendirilmiştir. vHİT test sonuçlarında da anlamlı bir fark elde edilmemiştir. LT4 tedavisi alan ve almayan HT olan hastalar arasında vestibuler testler açısından anlamlı farklılık izlenmemiştir. Ancak hipotiroidi (TSH>4,9) olan hastaların, vestibüler testleri, ötiroidi hastalarla karşılaştırıldığında, sağ ve sol lateral sakkad pozitifliğinin, hipotiroidide daha fazla olduğu görülmüştür (p=0,018, p=0,011). Her ne kadar diğer kanallar için gruplar arasında bir farklılık saptanmamış olsa da hipotiroidinin vestibüler fonksiyonlara olası etkileri açısından bu bulgumuzun önemli olduğunu düşünmekteyiz. Hashimoto thyroiditis is a common autoimmune thyroid disease and it is claimed that it may have systemic effect in any organ including inner ear. In the studies, increased frequency of autoimmune thyroid disease in patients with Meniere's disease and BPPV was found, and autoimmunity was implicated in the etiopathogenesis of both diseases. The aim of this study was to evaluate the effect of levothyroxine treatment and vestibular function in patients with Hashimoto thyroiditis. The study was conducted between March 2018 and August 2018 in the Başkent University Medical Faculty Endocrinology and Metabolic Diseases and Otorhinolaryngology polycnics. A total of 61 patients with HT diagnosis and 30 healthy individuals between 18 and 60 years of age from both genders, with no complaints of vestibular system, such as dizziness, tinnitus and hearing loss, were included in the study. Patients were evaluated in 2 groups; receiving levothyroxine treatment (30 patients) and not receiveing (31 patients). Free T3, free T4, TSH, anti-TPO and anti-Tg levels of all individuals were measured and cVEMP, oVEMP and vHIT tests were performed in Vestibüler Laboratory of Başkent University Otorhinolaryngology polyclinic. In the study, abnormalities in cVEMP tests were more common in patients with HT, however, we found no significant difference between groups. oVEMP tests were equally abnormal in both groups. However, we did not obtain any significant difference in oVEMP results. In our study all three semicircular canals were evaluated with vHIT tests. There is no significant difference in vHIT test results. There was no significant difference in vestibular tests between patients with HT receiving or not receiving levothyroxine treatment. However, vestibular tests of patients with hypothyroidism (TSH>4.9) compared with euthyroid patients showed right and left lateral saccade positivity were more in hypothyroidism (p=0.018, p=0,11). Although no difference was found between the groups for other canals, we think that this finding is important for the possible effects of hypothyroidism on vestibular functions.en_US
