dc.description.abstractBöbrek ve idrar yolunun konjenital anomalileri (CAKUT), antenatal olarak teşhis edilen tüm fetal anomalilerin %20-50'sini temsil eden, en sık tanımlanan malformasyonlardır. CAKUT, böbrek, toplayıcı sistem, mesane ve üretranın yapısal ve fonksiyonel anormallikleriyle karakterizedir. CAKUT, böbrek agenezisi, böbrek displazisi, hidronefroz, hidroüreter ve vezikoüreteral reflü gibi idrar yolu morfogenezindeki bozukluklardan kaynaklanan malformasyonları kapsar. Son yıllarda, 50'den fazla gendeki değişikliklerin CAKUT'a neden olduğu gösterilmiştir. Ancak bu genlerdeki mutasyonlar, CAKUT vakalarının sadece %10-20'sini açıklamaktadır. Bu veriler CAKUT’ta genetik heterojenite görüldüğünü göstermektedir. Bir düzenleyici protein olan BCL-2 (B hücreli lenfoma 2), insanlarda BCL-2 geni tarafından kodlanır. BCL-2 protein ailesi, apoptozun düzenlenmesinde etkin rol alır. BCL-2'nin kendisi, farklı tümör tiplerinde hem bir onkogen hem de bir tümör baskılayıcı gen olarak hareket eder. Bu nedenle bu gen kanser gelişimi, immün yanıt ve erken embriyonik gelişim gibi çok farklı mekanizmalarda görev alır. BCL-2’nin hücre ölümünü engellediğinden birçok çalışmada bahsedilmiştir. Bu çalışmalar, BCL-2 proteininin CAKUT ile ilişkili olduğunu düşündürmektedir. Lösemi inhibitor faktörü (LIF), hücre büyümesi ve hücre farklılaşmasını inhibe ederek etkisini gösteren interlökin-6 sınıfı (IL-6) bir sitokindir. LIF, blastokist büyümesi ve gelişimi, implantasyon için uterus hazırlığı gibi süreçlere aracılık eder. LIF, endometriyumun epitel hücrelerinde ifadelenir ve döllenmeden sonraki ilk günlerdeki LIF ifadelenmesi, blastokist implantasyonu için uygun ortam oluşturulmasında rol oynar. LIF geni, MAPK, PI3K, JAK/STAT gibi çeşitli sinyal yolağında rol alan önemli bir gendir. Böbrek gelişiminde JAK/STAT sinyal yolağı önemli bir rol oynamaktadır. LIF geninin JAK/STAT, MAPK, PI3K gibi sinyal yolaklarında görev alması, bu genin CAKUT ile ilişkili olduğunu düşündürmektedir. Bu çalışma kapsamında, BCL-2 geni 5’ kodlanmayan bölgesinde (UTR) bulunan rs2279115, 3’ UTR’de bulunan rs4987856 ve LIF geni 3’ UTR’de bulunan rs929271 tek nükleotit polimorfizmlerinin (SNP) CAKUT tanısı alan hastalardaki görülme sıklıkları araştırıldı. rs2279115 ve rs929271 SNP’leri PZR-RFLP, rs4987856 ise PZR-SNaPshot yöntemi ile çalışıldı. Proje kapsamında 0-18 yaş arası 129 CAKUT tanılı hasta ve 105 kontrol örneği kullanıldı. İstatistiksel analizler, incelenen polimorfizmlerin sıklıklarının, CAKUT tanısı alan hastalarda kontrollere göre istatistiksel olarak anlamlı derecede farklı olduğunu gösterdi. Çalışma sonucunda BCL2 geni rs2279115 ve rs4987867 SNP’leri ve LIF geni rs929271 SNP’i için hasta grubunda bulunan mutant allel frekansları istatistiksel olarak anlamlı bulunmuştur (p=0,000). Ayrıca, yapılan ikili analizlerde BCL2 geni rs4987856 SNP’inde atasal allel G’nin A alleline dönüşmesi ve LIF geni rs929271 SNP’inde atasal allel T’nin G’ye dönüşmesinin hasta ve kontrol grupları arasındaki dağılımı anlamlı bulunmuştur (p=0.034). Congenital anomalies of the kidney and urinary tract (CAKUT) are the most frequently defined malformations representing 20-50% of all fetal anomalies diagnosed antenatally. CAKUT is characterized by structural and functional abnormalities of the kidney, collecting system, bladder, and urethra and includes structural malformations caused by disorders in urinary tract morphogenesis such as renal agenesis, renal dysplasia, hydronephrosis, hydroureter, and vesicoureteral reflux. Recently, more than 50 genes have been shown to cause CAKUT. However, mutations in these genes explain only 10-20% of CAKUT cases. These data show that there is genetic heterogeneity in CAKUT. BCL-2 is encoded by the BCL-2 gene. The BCL-2 family of proteins plays an active role in regulation of apoptosis. BCL-2 itself acts as both an oncogene and a tumor suppressor gene in different tumor types. Therefore, this gene is involved in many different mechanisms such as cancer development, immune response, and early embryonic development. In addition, it has been shown that BCL-2 prevents cell death. Leukaemia inhibitory factor (LIF) is the interleukin-6 class (IL-6) cytokine that affects cell growth by inhibiting cell differentiation. LIF mediates processes such as blastocyst growth and development, and uterine preparation for implantation. Epithelial cells of the endometrium express LIF and this expression in the first days after fertilization creates a suitable environment for blastocyst implantation. Signaling via the JAK/STAT, MAPK pathways is important for the kidney's response to injury in disease. The fact that LIF gene plays a role in signaling pathways such as JAK / STAT, MAPK, PI3K suggests that this gene is related to CAKUT. In this study, three different SNPs of BCL-2 and the LIF genes were studied in patients with CAKUT. rs2279115 and rs4987856 SNPs in the BCL-2 gene are located in the 5'UTR and 3'UTR, respectively. rs929271 is located in the 3'UTR of the LIF gene. rs2279115 and rs929271 were analyzed by PCR-RFLP, and rs4987856 was analyzed by PCR-SNaPshot method. A total of 129 CAKUT patients between 0-18 years old, and 105 control individuals were enrolled in the study. As a result of the study, the distribution of mutant allele frequencies in the patient group for the BCL2 gene rs2279115 and rs4987867 and the LIF gene rs929271 were found to be statistically significant (p=0.000). In addition, cosegregation analysis showed A allele in the BCL2 gene for rs4987856 and G allele in the LIF gene for rs929271 cosegregating together in the patient group significantly (p=0.034).en_US
