Primer hipotiroidizm ve subklinik hipotiroidizmde serum total antioksidan kapasite ve lipid peroksidasyon belirteci malondialdehid (MDA) düzeyleri

Vücutta serbest oksijen radikallerinin (SOR) başlıca üretim yeri mitokondridir.
SOR lipid, karbonhidrat ve proteinler gibi hücre yapıtaşlarının oksidasyonuyla
organ fonksiyon bozukluklarına yol açabilir. Normal şartlarda bir yandan SOR
oluşurken, bir yandan da antioksidan sistemler tarafından ortadan kaldırılır.
Oksidatif stres, SOR’nin oluşumunda artış ya da antioksidan savunmanın
yetersizliğinin bir sonucu olabilir. Tiroid hormonlarının mitokondriyal oksijen
tüketimi üzerinde önemli etkileri olduğu bilinmektedir. Ancak hipotiroidizmde
oksidatif stresin artıp artmadığı ile ilgili veriler çelişkilidir. Subklinik
hipotiroidizmde ise oksidatif stresle ilişkili veriler çok daha azdır. Serum total
antioksidan status (TAS), antioksidan sistemlerin bütünü konusunda bilgi veren
bir parametredir. Malondialdehid (MDA) ise bir lipid peroksidasyon belirteci
olup, oksidatif strese bağlı peroksidasyonun değerlendirilmesinde
kullanılmaktadır. Bu çalışmanın amacı hipotiroidizm ve subklinik
hipotiroidizmde, sağlıklı kontrollerle karşılaştırıldığında serum TAS ve MDA
düzeylerinin nasıl etkilendiğini göstermektir. Çalışmaya 20 hipotiroid, 40
subklinik hipotiroid ve 40 sağlıklı kontrol alınmıştır. Hastaların serum TAS,
MDA, lipid profili ve C- reaktif protein düzeyleri çalışılmıştır. Çalışmamızda
hem hipotiroid hem de subklinik hipotiroid grupta MDA yüksek bulunurken, TAS
düzeyi her üç grupta da benzer bulunmuştur. Kontrol grubu ile karşılaştırıldığında
hipotiroid grupta LDL kolesterol ve trigliserit düzeyleri, subklinik hipotiroid
grupta ise LDL düzeyi yüksek bulunmuştur. MDA ile total kolesterol, LDL
kolesterol ve trigliserit düzeyleri arasında pozitif korelasyon saptanmıştır. Bu
sonuçlar hem hipotiroidizm hem de subklinik hipotiroidizmde oksidatif stresin
arttığını gösterirken, oksidatif ürünleri dengelemekte yetersiz kalan antioksidan
sistemin tek başına bundan sorumlu olmayabileceğini, değişen lipid
metabolizmasının oksidatif strese katkıda bulunan bir faktör olabileceğini
göstermiştir.
Mitochondria is the main production site of free oxygen radicals. These
oxygen species may lead to organ dysfunctions by oxidation of macromolecules
such as carbonhydrate, lipid and proteins. Oxygen species are produced during the
course of normal physiology, while the antioxidants eliminate them. An oxidative
stres may be a result of either overproduction of these species, or failure of the
antioxidant defence systems. Thyroid hormones have well known effects on the
mitochondrial oxygen consumption. There is challenging data about how does
hypothyroidism effect the oxidative stress. There is only a few data about the
oxidative stress in subclinical hypothyroidism. Serum total antioxidant status
(TAS) is a parameter which give information about all of the antioxidants.
Malondialdehide (MDA) is a lipid peroxidation marker which used to assess the
lipid peroxidation due to an increased oxidative stress. In this study we aimed to
find out how do hypothyroidism and subclinical hypothyroidism effect serum
MDA and TAS.
Hypothyroid 20, subclinical hypothyroid 40 and healthy 40 subjects were
included in the study. Serum TAS, MDA, C-reactive protein levels and lipid
compositions were studied.
MDA was elevated in both hypothyroid and subclinical hypothyroid
patients. TAS levels were not comparable. LDL cholesterol levels were
signifficantly high in both hypothyroid and subclinical hypothyroid patients, while
triglyceride levels were high in only hypothyroid patients when compared with
the controls. There was a positive correlation between MDA and LDL
cholesterol, total cholesterol and triglyceride levels. These results suggest that,
there is an increased oxidative stress in both states of hypothyroid and subclinical
hypothyroidism, which can be explained by both the insufficent increase in the
antioxidant status and the altered lipid meatbolism in these cases.
