Primer açık açılı glokom ve oküler hipertansiyonda rarebit, beyaz humphrey ve kısa dalga boyu humphrey görme alanlarının karşılaştırılması ve görme alanı ile optik sinir anatomisi ilişkisinin değerlendirilmesi
Özet
Bu çalısma, Rarebit perimetrisinin primer açık açılı glokom (PAAG) ve oküler
hipertansiyon (OHT) hastalarında Humphrey görme alanı testi ile uyumunun saptanması
amacıyla yapılmıstır. Ayrıca OHT hastalarında erken glokomatöz defekt
değerlendirilmesinde, Rarebit perimetrisinin kısa dalga boyu Humphrey görme alanı
(KDBHGA) testine üstünlüğünün olup olmadığının belirlenmesi, Heidelberg Retina
Tomografisi (HRT) III’deki topografik parametrelerle Rarebit ve Humphrey görme alanı
uyumunun saptanması ve HRT III’ün erken glokom tanısındaki yerinin belirlenmesi
amaçlanmıstır.
Çalısmaya 22 PAAG olgusunun 44 gözü, 21 OHT olgusunun 42 gözü ve 10 normal
olgunun 20 gözü dahil edildi. Hastalara oftalmolojik muayene, beyaz-beyaz Humphrey
görme alanı (BBHGA) Swedish interactive testing algorithm (SITA) standart programı,
KDBHGA SITA standart programı ve Rarebit perimetrisi iki kez uygulandı. Optik sinir
anatomileri HRT III ile incelendi. Görme alanı sonuçları ile HRT III verileri üç grupta
karsılastırıldı.
Primer açık açılı glokom, OHT ve kontrol grubunda, Rarebit görme alanı parameterleri ile
KDBHGA ve BBHGA parametreleri arasında anlamlı korelasyon tespit edilmedi (p>0.05).
Primer açık açılı glokom grubunda, KDBHGA ile BBHGA testleri arasında anlamlı
korelasyon saptandı (p<0.05).
PAAG ve OHT gruplarında HRT ile görme alanı testleri arasındaki korelasyonun bir kısım
parametrelerle sınırlı olduğu görüldü.
PAAG ile OHT grupları ve PAAG ile kontrol grupları karsılastırıldığında HRT
parametrelerinde ve görme alanı parametrelerinde anlamlı fark bulundu (p<0.05).
Sonuç olarak; Rarebit görme alanı ile klasik otomatize görme alanları arasında uyum tespit
edilememistir. Bunun nedeni Rarebit görme alanının tamamen farklı prensiplere dayanması
olabilir. HRT III ile elde edilen topografik verilerle en uyumlu görme alanı testinin
KDBHGA olduğu saptanmıstır. KDBHGA testi ile optik sinir bası parametrelerinin birlikte
değerlendirilmesi, erken glokom tanısı konulmasında daha etkili bulunmustur.
This study was carried out to evaluate the compatibility between Rarebit perimetry and
Humphrey visual field test in primary open angle glaucoma (POAG) and ocular
hypertension (OHT) patients, and to compare the diagnostic performance of Rarebit
perimetry and short-wavelength automated perimetry (SWAP) in OHT patients for
detecting early glaucomatous damage. Also we aimed to evaluate concordance between
results of the topographic parameters of the Heidelberg Retina Tomography (HRT) III and
Rarebit and Humphrey perimetry, and to find out the diagnostic performance of HRT III in
early glaucoma cases.
Forty four eyes of 22 POAG patients, 42 eyes of 21 OHT patients and 20 eyes of 10
normal patients were included to the study. All patients underwent ophthalmologic
examination, white on white Humphrey visual field test (WWHVFT) Swedish interactive
testing algorithm (SITA) standard programme, SWAP SITA and Rarebit perimetry for two
times. HRT III was used in the analysis of optic nerve anatomy. The visual field results and
HRT III parameters were compared in three groups.
There was no significant correlation between Rarebit perimetry and WWHVFT or SWAP
in POAG, OHT and control groups (p>0.05). In POAG group, statistically significant
correlations were found between WWHVFT and SWAP (p<0.05).
In POAG and OHT groups, correlations between HRT and visual field tests were limited to
some of the parameters.
When POAG group was compared with OHT group and control group, significant
difference was found in HRT parameters and visual field test indices (p<0.05)
In conclusion; Rarebit perimetry and standard automated perimetry were not found to be
compatible. The reason for this finding may be the different working principles of Rarebit
perimetry. Topographic parameters obtained from HRT III were mostly correlated with
SWAP. The combination of SWAP test and topographic optic nerve head analysis could
improve early glaucoma diagnosis.
