Başkent Üniversitesi Ankara Hastanesi erişkin acil servisine 2011 - 2014 yıllarında zehirlenme ile başvuran hastaların özellikleri ve maliyet analizi

Bu çalışmada Başkent Üniversitesi Ankara Hastanesi Erişkin Acil servisine
zehirlenme sebebiyle getirilen hastaların, zehirlenme şekillerinin, hospitalizasyon
oranlarının ve maliyet analizinin araştırılması amaçlandı.
Çalışmamız; etik kurulu onayı alındıktan sonra, Başkent Üniversitesi Ankara
Hastanesi Acil Anabilimdalı’ında, 01.01.2011 – 31.12.2014 tarihleri arasında zehirlenme
sebebiyle başvuran 646 hasta ile retrospektif olarak yapıldı. Hastaların yaş, cinsiyet,
başvuru tarihi, kesin tanıları, yatırılma oranları, sosyal güvenceleri ve maliyetleri incelendi.
Değişkenlerin normal dağılımı Kolmogorov Simirnov testi ile test edildi. Sayısal
non-parametrik verilerin analizinde Mann-Whitney-U ve Kruskal-Wallis H testleri;
niteliksel verilerin analizinde Ki-kare testi, sayısal verilerin karşılaştırılmasında Pearson
korelasyonu kullanıldı. p<0,05 değeri istatistiksel olarak anlamlı kabul edildi.
Hastaların yaş ortancasının 29 yıl ve %67,8’si kadındı. Hastaların cinsiyete göre
yaşları arasında istatistiksel olarak anlamlı bir ilişkiye rastlanmadı (p>0,05). Hastaların en
sık ilaç intoksikasyonuyla (%40,6) başvurduğu görüldü. Alkol ve ilaç intoksikasyonu
sebebiyle getirilen hastaların yaş ortancası düşük iken, gıda intoksikasyonu sebebiyle
getirilen hastaların yaş ortancasının yüksek olduğu saptandı (p<0,05). Kadın hastaların ilaç
alımına bağlı intoksikasyonlarının daha sık olduğu saptandı (p<0,05). Hastaların en sık kış
aylarında olduğu ve hasta sayısının yıllara göre artığı saptandı (p<0,05). Hastaların sosyal
güvencesi arasında anlamlı bir ilişki bulunamadı (p>0,05). Hastaların %96,1’i ayaktan
tetkik ve tedavi edilirken; cinsiyet hospitalizasyon oranları arasında istatistiksel olarak
anlamlı bir ilişkiye rastlanmadı (p>0,05). İlaç ve kimyasal madde ile zehirlenenlerde
hospitalizasyon oranı yüksek saptandı (p<0,05). Hastaların maliyet ortancası 75,14 tl’ydi.
Çalışma grubumuzda hastaların yaş ve maliyet arasında doğrusal bir ilişki saptandı
(p<0,05). Kadın hastaların, ilaç intoksikasyonu sebebiyle getirilen hastaların ve taburcu
edilen hastaların maliyeti anlamlı olarak yüksekti (p<0,05).
Sonuç olarak acil serviste zehirlenme sebebiyle getirilen hastaların ilaç
intoksikasyonu olması sebebiyle, uygun sosyal destek ile zehirlenme olgu sayısının
azaltılabileceği düşüncesindeyiz. Ayrıca zehirlenme olgusunu yöneten ekip içerinde
bulunacak deneyimli kişilerin maliyeti azaltabileceği görünmektedir.
In this study, we aimed to investigate forms of intoxication, the hospitalization rates
and cost analysis of patients who admitted to Baskent University Ankara Hospital
Emergency Department because of poisoning.
Our study; was performed retrospectively with 646 patients who admitted to
Baskent University Ankara Hospital Emergency Department because of poisoning between
01.01.2011 – 31.12.2014 after obtaining the approval of the ethics committee. Age,
gender, date of admission, final diagnosis, hospitalizing rates, social insurance and hospital
costs of patients were examined.
The normal distribution of the variables was tested with Kolmogorov Smirnov test.
Mann-Whitney U and Kruskal-Wallis H tests were used for analysing quantitative nonparametric
data. Chi-square test was used in the analysis of qualitative data and Pearson's
correlation was used for the comparison of numerical data. Value of p <0.05 was accepted
as statistically significant.
The median age of the patients was 29 years and 67,8 % were female. There was no
significant relationship between the ages of patients and gender (p> 0.05). It was seen taht
the most common reason for admission was drug intoxication (%40,6) . While the median
age of patients admitted due to alcohol and drug intoxication was low, the median age of
the patients due to food poisoning was found to be high (p <0.05). Intoxication depending
on the drug intake was higher in female patients (p <0.05). It was determined that number
of patients was higher in winter and increased over the years (p<0,05). Patients were
determined to be registered to the most common social insurance institutions. There was
not a significant relationship between social insurance and poisoning form of patients (p>
0.05). While 96.1% of the cases were examined and treated as outpatients; there was no
statistically significant relationship between gender and hospitalization rates (p> 0.05). The
rate of hospitalization in patients with poisoning by drugs and chemicals were higher (p
<0.05). The median cost of the patients was 75.14 TL. Our study revealed a linear
relationship between patient age and costs (p <0.05). Cost of female patients, drug
intoxication patients and discharged patients was significantly higher (p<0,05).
As a result, due to the intoxication of patients admitted due to drug poisoning in the
emergency department, we believe the number of poisoning cases can be reduced with
proper social support. Moreover, the presence of an experienced person in the team that
manages the poisoning cases also appears to reduce cost
