Alfa lipoik asitin gürültüye bağlı işitme kaybına karşı koruyucu etkisinin araştırılması
Abstract
Gürültüye bağlı işitme kaybı dünya çapında erişkin popülasyonun işitme kaybının
en önemli nedenlerinden biridir. Gürültüye bağlı işitme kaybı orta kulak ve iç kulak
yapılarında mekanik hasara yol açabilmekte, koklear kan akımını bozmakta kokleada
reaktif oksijen moleküllerini artırmaktadır. Birçok antioksidan molekülün gürültüye bağlı
işitme kaybında koruyucu etkisi tesbit edilmiştir. Alfa lipoik asit diabetik nöropatide
kullanılan güvenilir bir antioksidandır ve nöroprotektif etkilidir. Çalışmamız gürültü
maruziyeti sonrası oluşan otolojik hasara karşı alfa lipoik asitin koruyucu etkisinin test
edildiği in vivo kontrollü deneysel bir hayvan çalışmasıdır. Bu çalışma Başkent
Üniversitesi Tıp ve Sağlık Bilimleri Araştırma Kurulu tarafından onaylanmıştır ve Başkent
Üniversitesi hayvan deneyleri laboratuvarında yürütülmüştür. Çalışmaya 24 rat dahil
edilmiş, birinci grup sadece gürültü, ikinci grup sadece alfa lipoik asit, üçüncü grup gürültü
ve alfa lipoik asit verilen, dördüncü grup da kontrol grubu olacak şekilde ayarlanmıştır.
Tüm ratların bazal ABR ölçümleri yapıldıktan sonra alfa lipoik asit grubu ve alfa lipoik
asit ve travma grubuna 24 gün boyunca tek doz salin içinde çözünmüş 300mg/kg lipoik asit
hergün aynı saatte gavaj yoluyla verilmiştir, yine kontrol grubuna da 300mg/kg salin gavaj
yoluyla uygulanmıştır. Bazal ölçümden 3 gün sonra travma ve travma ile birlikte lipoik asit
alan iki gruba 107db SPL şiddetinde beyaz gürültü serbest alanda uygulanmıştır. Gürültü
maruziyeti sonrası 1. ve 21.gün tüm grupların ABR ölçümleri yapılmıştır. Travma ve
travma+ilaç alan grupta gürültü sonrası eşik değerleri yükselmiştir. (p<0.05) Travma
grubunda eşik yükseklikleri 21.gün ölçümlerinde devam ederken,travma+ilaç grubunda
anlamlı düzelme görülmüştür. (p<0.05) Sonuç olarak alfa lipoik asitin gürültüye bağlı
ototoksiteye karşı ratlarda tedavi edici etkisi bulunmuştur. Alfa lipoik asit gürültüye bağlı
işitme kaybında alternatif bir tedavi olarak kullanılabilir.
Noise-induced hearing loss is one of major causes of hearing loss in the adult
population. Reactive oxygen levels increases in cochlea after noise and cochlear blood
flow is impaired. The protective effects of many antioxidant treatments were determined in
noise-induced hearing loss.Alfa lipoic acid which protect the body from diabetic
neuropathy has also antioxidant, and neuroprotective effect. This study tested protection of
alfa lipoic acid against noise induced ototoxicity as an in vivo controlled experimental
animal study. This study was approved by Başkent University Medical and Health
Sciences Research Committee and was carried out in animal laboratory of Baskent
University. Twenty four rats were included in the study; groups were set as: The first group
received alfa lipoic acid, the second group control group, the third group was exposed to
noise and received alfa lipoic acid, the fourth group exposed to noise. After baseline ABR
measurements of all rats, alfa lipoic acid group (group 1) and alfa lipoic acid + noise group
(group 3) received 300 mg/kg alfa lipoic acid (tiopati®) treatment dissolved in saline as
single dose in a day by using gavage for 23 days. Noise group (group 4) and alfa lipoic
acid + noise group (group 3) were exposed to 107 dB SPL white noise for 15 hours at 3th
day. ABR measurements of the rats in all groups were repeated at 1st, 21st days after noise
exposure. The rats in 1nd and 3rd groups continued to receive alfa lipoic acid treatment for
23 days until the end of the study. At the first day after noise exposure, ABR thresholds
were deteriorated in 3 rd and 4 th groups, deterioration continued at 21st days in the fourth
group, deterioration returned to normal at 21 st day in the third group. There were no
changes in ABR thresholds with alfa lipoic acid treatment in alfa lipoic acid group. As a
result, it was found that alfa lipoic acid has therapeutic effect on noise-induced ototoxicity
in rats. Alfa lipoic acid can be used as an alternative treatment of noise induced hearing
loss.
