Bariatrik ve metabolik cerrahi geçirmiş hastaların antropometrik ölçümleri ve biyokimyasal parametrelerinin değerlendirilmesi
Özet
Bu çalışma, bariatrik ve metabolik cerrahi geçirmiş bireylerin cerrahi öncesine göre, cerrahi sonrasındaki antropometrik ölçümlerinin ve biyokimyasal parametrelerinin değişimini belirlemek amacıyla yürütülmüştür. Çalışma; Kasım 2017 – Ağustos 2020 tarihleri arasında Aksaray Üniversitesi Eğitim ve Araştırma Hastanesi’nde bariatrik cerrahi geçiren 102, metabolik cerrahi geçiren 13 hasta olmak üzere toplam 115 hasta ile yapılmıştır. Bireylerin demografik özellikleri, genel sağlık durumları, antropometrik ölçümleri ve biyokimyasal parametreleri Aksaray Üniversitesi Eğitim ve Araştırma Hastanesi Hastane Bilgi Yönetim Sistemi ve Postoperatif Hasta Değerlendirme Formu incelenerek araştırmacı tarafından kaydedilmiştir. Çalışmaya katılan bireylerin %20.6’sı erkek, %79.4’ü kadındır. Bariatrik cerrahi geçiren erkek hastaların yaş ortalaması 33.3±8.78 yıl, kadın hastaların ise 35±10.77 yıl olarak saptanmıştır. Metabolik cerrahi geçiren erkek hastaların yaş ortalaması 64±1.41 yıl kadın hastaların yaş ortalaması 52.2±7.77 yıl olarak belirlenmiştir. Bariatrik cerrahi geçiren erkek hastaların cerrahi öncesine göre, cerrahi sonrası 3. aydaki ağırlık kaybı oranları ortalama %24.2±6.11 kg olarak; cerrahi öncesine göre, cerrahi sonrası 6. ayda ağırlık kaybı oranları ortalama %32.4.±6.77 kg olarak hesaplanmıştır. Bariatrik cerrahi geçiren kadın hastaların cerrahi öncesine göre, cerrahi sonrası 3. aydaki ağırlık kaybı oranları ortalama %21.9±5.09 kg olarak; cerrahi öncesine göre, cerrahi sonrası 6. ayda ağırlık kaybı oranları ortalama %30.6±5.24 kg olarak hesaplanmıştır. Bariatrik cerrahi geçiren erkek ve kadın hastalar için cerrahi öncesine göre cerrahi sonrası 3. ve 6. ay ile cerrahi sonrası 3. aya göre cerrahi sonrası 6. ay vücut ağırlıklarının istatistiksel açıdan önemli olarak azaldığı belirlenmiştir (p<0.05). Metabolik cerrahi geçiren erkek hastaların cerrahi öncesine göre, cerrahi sonrası 6. aydaki ağırlık kaybı oranları ortalama %24.8±7.38 kg olarak hesaplanmıştır. Metabolik cerrahi geçiren kadın hastaların cerrahi öncesine göre, cerrahi sonrası 3. aydaki ağırlık kaybı oranları ortalama %6.8±3.89 kg; cerrahi öncesine göre, cerrahi sonrası 6. aydaki ağırlık kaybı oranların ortalama %27.7±7 kg olarak hesaplanmıştır.Metabolik cerrahi geçiren kadın hastaların cerrahi öncesine göre cerrahi sonrası 3. ve 6. ay ile cerrahi sonrası 3. aya göre cerrahi sonrası 6. ay vücut ağırlıklarının istatistiksel açıdan önemli olarak azaldığı tespit edilmiştir (p<0.05). Bariatrik cerrahi geçiren erkek hastalarda cerrahi öncesine göre cerrahi sonrasında açlık plazma glukozu düzeylerinin değişmediği (p>0.05) fakat hem bariatrik hem de metabolik cerrahi geçiren kadın hastalar için cerrahi öncesine göre, cerrahi sonrası 3. ve 6. aydaki açlık plazma glukozunun azaldığı saptanmıştır (p<0.05). Bu çalışmada, bariatrik cerrahi geçiren hem erkek hem de kadın hastaların ve metabolik cerrahi geçiren kadın hastaların cerrahi öncesine göre cerrahi sonrası 3. ve 6. aydaki serum insülin düzeylerinin azaldığı belirlenmiştir (p<0.05). Sonuç olarak, bu çalışma bariatrik ve metabolik cerrahi geçiren hastaların antropometrik ölçümleri ve biyokimyasal parametrelerinin değişimini belirleyip, cerrahiden sonra oluşabilecek komplikasyonları değerlendirerek obezite tedavisinde cerrahi yöntemin uygulanmasının olası sonuçlarını ortaya koymuştur.
The study was carried out to determine the change of anthropometric measurements and biochemical parameters after surgery compared to pre-surgery of individuals who had bariatric and metabolic surgery. The study was conducted with a toplam of 115 patients, 102 of whom underwent bariatric surgery and 13 patients who underwent metabolic surgery, at Aksaray University Training and Research Hospital between November 2017 and August 2020. The demographic characteristics, general health status, anthropometric measurements, and biochemical parameters of the individuals were recorded by the researcher by examining the Aksaray University Training and Research Hospital Information Management System and Post-surgery Patient Evaluation Form. 20.6% of the individuals participating in the study were male and 79.4% were female. The mean age of male patients who underwent bariatric surgery was 33.3±8.78 years, and 35±10.77 years in female patients. The mean age of male patients undergoing metabolic surgery was 64±1.41 years, and the mean age of female patients was 52.2±7.77 years. The average weight loss rate of male patients undergoing bariatric surgery was 24.2%±6.11 kg at the 3rd month after surgery compared to the pre-surgery period. The 6-month post-surgery weight loss rate was calculated as 32.4% ±6.77 kg on average compared to pre-surgery. The average weight loss rate of female patients undergoing bariatric surgery was 21.9%±5.09 kg at the 3rd month after surgery compared to pre-surgery and 6-month post-surgery weight loss rates were calculated as 30.6%±5.24 kg on average. For male and female patients who underwent bariatric surgery, it was determined that body weights decreased at 3 and 6 months after surgery compared to before surgery and also decreased at 6 months compared to 3 months after surgery (p<0.05). The average weight loss rate of male patients who underwent metabolic surgery was calculated as 24.8%±7.38 kg at the 6th month after surgery compared to pre-surgery. The average weight loss rate of female patients who underwent metabolic surgery was 6.8%±3.89 kg at the 3rd month after surgery compared to pre-surgery and the mean weight loss rates at the 6th month after surgery were calculated as 27.7%±7 kg. It was determined that the body weights of the female patients who underwent metabolic surgery decreased at the 3rd and 6th months after surgery compared to the pre-surgery period, and at the 6th month compared to the 3rd month after surgery (p<0.05). In male patients who underwent bariatric surgery, fasting plasma glucose levels did not change after surgery compared to pre-surgery (p>0.05), but for female patients who underwent both bariatric and metabolic surgery, fasting plasma glucose levels decreased at the 3rd and 6th months after surgery compared to pre-surgery (p>0.05). In this study, it was determined that the serum insulin levels of both male and female patients undergoing bariatric surgery and female patients undergoing metabolic surgery decreased at the 3rd and 6th months after surgery compared to pre-surgery (p<0.05). In conclusion, this study determined the anthropometric measurements and changes in biochemical parameters of patients undergoing bariatric and metabolic surgery and evaluated the complications that may occur after surgery, revealing the possible results of the surgical method in the treatment of obesity.
