dc.description.abstractLösemi, kemik iliği tarafından üretilen beyaz kan hücrelerinin kanseridir. Lösemi akut ve kronik olmak üzere iki gruba ayrılır. Kronik miyeloid lösemi (KML) tüm yetişkin lösemilerin %15’ini oluşturur. Yirmikinci kromozom ile 9. kromozom arasındaki karşılıklı parça değiş tokuşu sonucunda yeni oluşan 22. kromozom Philadelphia kromozomu (Ph) olarak adlandırılır. İlk olarak 1960 yılında KML'li bir hastada tanımlanmıştır. BCR-ABL1 füzyon geni KML patogenezinde önemli rol oynayan tirozin kinaz aktivitesine sahip bir proteindir. Bu onkoproteinin ifadelenmesi, hücre sağ kalımını ve çoğalmasını arttıran sinyal yolağının daha alt basamaklarında yer alan sinyal proteinlerini aktive ederek hematopoetik progenitör hücreleri dönüştürür. Bu çalışmanın amacı BCR-ABL1 füzyon geni varlığını CRISPR/Cas9 sistemini kullanarak saptamaktır. Bu kapsamda BCR-ABL1 füzyon genine sahip iki KML hastasının komplementer DNA (cDNA)’sı, polimeraz zincir tepkimesi (PZR) yöntemi ile çoğaltıldı. Füzyon bölgesini saptamak amacıyla Sanger dizileme yapıldı ve BCR-ABL1 füzyon bölgesinin dizisi çıkarıldı. Bu çalışmada, hastalardan birinden elde edilen BCR-ABL1 füzyon genini hedefleyen kılavuz RNA dizayn edildi. Cas9 endonükleaz ve kılavuz RNA birlikte kullanılarak laboratuvar ortamında BCR-ABL1 füzyon bölgesinin belirlenip, kesim yapılması sağlandı. Yapılan tez çalışması sonucunda elde edilen verilere göre, füzyon dizisi tanıma hassasiyeti %2 olarak tespit edildi. Bu yöntem CRISPR/Cas9 yönteminin sadece genom düzenleme için değil aynı zamanda translokasyon tipi mutasyonlarının da tespit edilmesi amacıyla kullanılabileceğini göstermektedir. İleride yapılacak çalışmalarda testin hassasiyeti ve aynı anda tanıyabileceği füzyon noktalarının sayısının arttırabileceği öngörülmektedir. Leukemia is a cancer of white blood cells produced by the bone marrow. Leukemia is divided into two main groups: acute and chronic. Chronic myeloid leukemia (CML) accounts for 15% of adult leukemia. The Philadelphia chromosome (Ph), was first described in a patient with CML in 1960, had a reciprocal translocation between the proto-oncogene tyrosine-protein kinase (ABL1) gene on chromosome 9 to the breakpoint cluster region (BCR) gene on chromosome 22. The product of the BCR-ABL1 fusion gene is a protein with tyrosine kinase activity that plays an important role in the pathogenesis of CML. The characteristic expression of this oncoprotein transforms hematopoietic progenitor cells by activating downstream signaling cascades that increase cell survival and proliferation. This study aims to detect patients who have a BCR-ABL1 fusion gene with the CRISPR / Cas9 system. For this reason, two patients’ cDNAs who have the BCR-ABL1 fusion gene is amplified by polymerase chain reaction (PCR). Sanger sequencing was performed to detect the fusion region and its’ sequence. In this study, A guide RNA was designed for targeting the BCR-ABL1 fusion region of a patient’s cDNA. The fusion region detection sensitivity is calculated as 2%. The CRISPR / Cas9 method can be used not only for genome editing but also to detect translocation type gene rearrangement. In future studies, it is predicted that the sensitivity of the test and the number of fusion regions that can be detected at the same time might be increased.en_US
