Mandibuler onlay greft uygulamalarında kortikal perforasyonun kemik iyileşmesine olan etkisinin histolojik ve radyolojik olarak incelenmesi
Özet
Bu çalışma, mandibuler onlay greft uygulamalarında yapılan kortikal perforasyonların
kemik iyileşmesine etkisini radyolojik ve histolojik olarak incelemek amacıyla yapılmıştır.
Çalışmada 7 adet erişkin domuza kortikal otojen kemik greftleri ile agumentasyon
yapılmıştır. Domuzların alt çenelerinin sol tarafı deney, sağ tarafı ise kontrol grubu olarak
belirlenmiş, deney grubunda greft fiksasyonundan önce alıcı bölgenin kortikal kemiğine
perforasyonlar yapılmış, kontrol grubunda ise alıcı bölgeye herhangi bir perforasyon
yapılmadan otogreft fikse edilmiştir.
12 haftalık iyileşme periyodundan sonra domuzlar sakrifiye edilerek greft bölgeleri rezeke
edilmiş, rezeke edilen parçalarda önce radyolojik daha sonra da histopatolojik inceleme
yapılmıştır.
Radyolojik değerlendirmelerde deney ve kontrol grubu arasında anlamlı bir fark
bulunamamıştır (p>0,05). Histopatolojik incelemelerin sonucunda ise deney ve kontrol
grubu greftlerinin kalınlıkları arasındaki farkın önemli olduğu görülmüştür (p<0,05). Alıcı
bölgenin üst ve alt yarılarındaki remodelizasyon ve greftlerdeki osteoblastik aktivite
incelendiğinde deney grubu ile kontrol grubu arasında anlamlı bir fark bulunmamıştır
(p>0,05).
Sonuç olarak mandibuler onlay kemik grefti ile agumentasyon yapılırken alıcı bölgede
oluşturulan kortikal perforasyonların 12 haftalık dönemde kemik iyileşmesine belirgin
katkısının bulunmadığı görülmüştür.
This study was conducted to evaluate the effects of cortical perforations on bone healing in
mandibular onlay graft procedures radiologically and histologically.
In this study, augmentation with cortical autogeneous bone grafts were applied to 7 adult pigs.
The left and the right mandibular sides of the pigs were defined as the experimental and
control groups, respectively. On the experiemental group, before graft fixation, perforations
were performed in the recipient site cortical bone. On the control group, autograft fixation was
applied to the recipient site without any perforations.
After a healing period of 12 weeks, pigs were sacrified and grafted sites were resected.
Radiological and than histopatological evaluations were perfomed on these resected sites.
In the radiological evaluation, there was no statistically significant difference between the
experimental and the control groups (p>0.05). On the other hand, according to the results of
histological evaluation, there was a significant difference in the thickness of the grafts
between the experimental and the control groups (p<0.05). However, when the remodelization
on the upper and lower half of the recipient site and osteoblastic activity in the grafts were
carried out, no significant difference was observed between the experimental and the control
groups (p>0.05).
As a result, in augmentation procedure with mandibular onlay bone grafts, cortical
perforations in the recipient site have no distinctive contribution to bone healing in twelve
weeks.
