Tüberküloz plörezide serum ve plevral sıvı örneklerinde early secreted antigenic target-6 (ESAT-6) ve culture filtrate protein-10 (CFP-10) antijenlerine spesifik T-lenfositleri analizinin tanısal değeri
Abstract
Tüberküloz (TB) plörezi, Mycobacterium tuberculosis (MTB) infeksiyonunun en
sık görülen ekstrapulmoner tutulumudur. Tüm TB vakalarının ortalama %37.4’ünde
bulunur. Ülkemizin pek çok yerinde eksudatif plörezinin sebepleri arasında TB ilk sırada
yer alır. Ancak TB plörezi tanısında güçlükler vardır. Standart tanısal yöntemlerle plevral
sıvı analizi yaparak TB plörezi tanısı koymak zordur. TB plörezi düşünülen hastaların
tanısında daha az invaziv, hızlı, duyarlı ve özgül immunolojik tanı yöntemlerine
gereksinim vardır.
Bu araştırmada, plevral sıvı, plevral doku kültürü veya histolojik olarak TB plörezi
tanısı konmuş hastaların plevral sıvılarında ve kanlarında ELİSPOT yöntemiyle T
SPOT.TB testi çalışarak, testin TB plörezideki tanı değeri araştırıldı. Bu amaçla, TB
plörezi tanılı (n=10) ve konjestif kalp yetmezliğine bağlı transüdatif plevral efüzyonu olan
(n=20) toplam 30 olgunun dahil edildiği prospektif bir çalışma yürütüldü.
Transüdatif efüzyonu olan hastaların ESAT-6 ve CFP-10 antijenlerine spesifik Tlenfositleri
cevabında serum ve plevral sıvıda fark izlenmedi. Ancak TB plörezili
hastalarda plevra ESAT-6 ve CFP-10 antijenlerine verilen cevabın serum ESAT-6 ve CFP-
10 antijenlerine verilen cevaba göre anlamlı olarak yüksek olduğu izlendi (p<0.05).
Toplam hücre sayısında, transüdatif efüzyonu olan hastaların serum ve plevra
arasında fark saptanmazken TB plörezide serum ve plevral sıvıda kontrol grubuna gore
anlamlı yükseklik saptandı (p<0.05).
Sonuçlarımız, TB plörezide T-SPOT.TB testinin serum ve plevrada
uygulanarak MTB-spesifik T-lenfositlerin plevral sıvıdaki sayılarının
serumdaki sayılarına göre daha yüksek olduğunun gösterilmesinin TB
plörezi tanısına katkı sağlayacağını desteklemektedir. Ancak olgu sayısının
azlığı göz önüne alındığında bu konuyla ilgili yapılacak geniş çaplı
çalışmalara ihtiyaç duyulduğu söylenebilir
Tuberculous (TB) pleurisy is the most common extrapulmonary manifestation of
the Mycobacterium tuberculosis (MTB) infection, accounting for up to 37.4 percent of
tuberculosis cases. In our country it is the most frequent cause of exudative pleural
effusions. Although there are difficulties for the diagnosis of TB pleurisy. Because the
diagnosis of TB pleurisy by the analysis of pleural effusions with standard diagnostic tools
is difficult. The sensitivity for the detection of active TB infection is higher in pleural
biopsies compared to pleural fluid, although the procedure is invasive. When the frequency
and diagnostic difficulties of TB pleurisy are considered, its certain that a more sensitive,
specific and less invasive test for detection of TB pleurisy infection is required.
In this study our objective is to determine antigen specific T cells by using
ELISPOT assay on peripheral blood mononuclear and pleural effusion mononuclear cells
in patients with diagnosed TB pleurisy . We conducted a prospective study enrolling a total
of 30 participants with diagnosed TB pleurisy (n=10) and patients with transudative pleural
effusions due to congestive heart failure (n=20).
There was no difference between the specific T lymphocyte answers of serum and
pleural ESAT-6 and CFP-10 antigens in patients with transudative pleural effusions.
Although the answers of pleural ESAT-6 and CFP-10 antigens were higher then the
answers of serum ESAT-6 and CFP-10 antigens in patients with diagnosed TB pleurisy
(p<0.05).
The difference between total cell counts were not determined in serum and pleural
effusion in patients with transudative pleural effusions although it was significantly higher
in serum and pleural effusion in patients with diagnosed TB pleurisy (p<0.05).
The results of our study supports that enumerating effector T cells by ELISPOT at
the site of pleural effusion should enable more specific diagnosis of TB pleurisy than
enumerating effector T cells in the blood alone. However larger, prospective studies are
required to confirm these results.
